Fish in Pregnancy and Breastfeeding: Beware of Elevated Methylmercury Levels in Selected Species
Fish is a valuable source of essential nutrients, trace elements, and vitamins, which is why the German Nutrition Society (DGE) recommends consuming fish once or twice a week.
However, the organic form of mercury, known as methylmercury, can accumulate in fish through the marine food chain. As a result, certain fish species, particularly predatory fish, may have elevated levels of methylmercury. Methylmercury can have neurotoxic effects. Unborn children and infants are particularly sensitive to the neurotoxic effects, which is why there are special dietary recommendations for pregnant and breastfeeding women. The Federal Ministry for the Environment, Nature Conservation, Nuclear Safety and Consumer Protection (BMUV) provides consumer tips warning against the consumption of fish species with high methylmercury levels.
The Federal Institute for Risk Assessment (BfR) analyzed current mercury content data in fish in its statement No. 047/2023 dated October 11, 2023, and calculated the estimated intake of methylmercury for women of childbearing age. The results show that the choice of fish species is crucial. In particular, with shark and swordfish, the tolerable weekly intake (TWI) for methylmercury is exceeded when consuming one or two servings. Tuna remains well below the recommended two servings TWI but approaches it with three meals, especially at high levels. Fish species such as halibut and eel can also exceed the TWI for methylmercury.
Despite the potential risks, the BfR emphasizes that the health benefits of fish consumption should not be overlooked. Fish is an important source of vitamins, trace elements, and polyunsaturated fatty acids. However, the current assessment does not provide grounds for changing consumer tips on fish consumption for pregnant and breastfeeding women from the federal ministry.
Overall, it is recommended to carefully select fish species to utilize the positive effects of fish consumption while minimizing the potential risks from methylmercury, especially during pregnancy and breastfeeding.
